This rare honor places FoundCare among a select few community health centers nationally to receive such high recognition.

FoundCare, operating across 7 clinics throughout Palm Beach County with a Riviera Beach location soon to open, takes pride in this remarkable accomplishment. Every three years, Federally Qualified Health Centers undergo an OSV conducted by The US Department of Health & Human Services, Health Resources & Services Administration (HRSA), Bureau of Primary Health Care. The audit encompasses extensive evaluation across three major categories: Clinical/Quality, Governance/Administration, and Fiscal Management, covering every operational aspect from community leadership to healthcare risk management.
