The Health Resources and Services Administration (HRSA) Quality Improvement Awards recognize the highest performing health centers nationwide, as well as those health centers that have made significant quality improvement gains from the previous year. Below are the awards FoundCare received for fiscal year (FY) 20.

Advancing HIT for Quality Award- FoundCare optimized HIT services, for advancing telehealth, patient engagement, interoperability, and collection of social determinants of health, to increase access to care and advance quality of care.

Disparities reducer

Health Disparities Reducer Award- FoundCare met or exceeded the Healthy People 2020 goals, or made at least a 10% improvement across different racial/ethnic groups.

Patient Centered Medical Home (PCMH) Recognition- The primary care medical home provides health care that is relationship-based with an orientation toward the whole person. Partnering with patients and their families requires understanding and respecting each patient’s unique needs, culture, values, and preferences. The medical home practice actively supports patients in learning to manage and organize their own care at the level the patient chooses. Recognizing that patients and families are core members of the care team, medical home practices ensure that they are fully informed partners in establishing care plans.
