    Online, please complete the Appointment Request Form and a member from our team will call you to schedule an appointment.

  • Do you accept walk-ins?

    Yes. Walk-ins are seen as quickly as possible. We strongly encourage everyone to make an appointment ahead of time.

  • Can I be seen if I don’t have health insurance?

    Yes. We offer a sliding fee scale based on family size and income for those who are not covered by insurance. You may ask us to conduct a financial assessment to determine your eligibility for our sliding fee scale. No one is turned away for inability to pay. Click here to learn more. 

  • Do your providers offer services in languages other than English?

    Yes. Our culturally competent staff is fluent in English, Spanish, Haitian Creole, and French.

  • What information should I review prior to my first visit / appointment (new patients)?

    Please review our Notice of Privacy Practices prior to your scheduled visit. This notice describes how your personal medical information may be used and disclosed and how you can get access to this information. Please review it carefully. English Spanish Creole

  • What information should I bring when I arrive for my first visit / appointment (new patients)?
    1. Photo ID (all patients)
    2. Uninsured patients, please provide income verification (pay stub, W2, or tax return)*
    3. Unemployed patients, please provide verification of Unemployment (SEQY) or Disability (TPQY) from Social Security
    4. Undocumented and uninsured patients must provide a Letter of Support (Valid To/From Dates, the contact information of the supporter, and be notarized)*
    5. Insurance card (if applicable)
    6. Past medical history or records

Our goal as a community health center is to provide comprehensive quality health care to our patients. Every patient seen by FoundCare providers receives the same high-quality standard of care regardless of whether or not they have insurance.
