    Mental health affects the way we feel, think, and behave, and it is also directly related to how we handle stress and relate to others. Countless factors can contribute to the development of a mental health concern, and because of this, behavioral health problems, both temporary and permanent, are common. The good news is that help is available, and many people improve their behavioral health with treatment.

    Primary care settings have become a place for individuals with behavioral health needs to receive treatment. FoundCare recognizes this and prides itself on being an integrated healthcare facility that reduces the stigma around seeking mental health care by offering behavioral health within a primary care setting. FoundCare’s Behavioral Health Team can provide services in English, Spanish, and Creole.

    Did you know that your oral health is directly linked to your overall health? Oral health is an essential component of health throughout life. Poor oral health and untreated oral diseases can have a significant impact on the quality of life. They can affect the most basic human needs, including the ability to eat, drink, swallow, maintain proper nutrition, smile, and communicate.

    At FoundCare, we are committed to providing you and your family with the highest quality of dental care possible through comprehensive, preventive, restorative, and emergency dental care in a warm and friendly setting.

    Our staff is fluent in English, French, Spanish, and Haitian Creole to ensure that you can communicate your needs to us. We provide both adult and pediatric dental care.

    A comprehensive dental exam will be conducted with X-rays upon your first visit. FoundCare Dental is only able to perform basic dental procedures. If we are unable to perform the services recommended by our Dentist, we will provide you with a referral to obtain the additional services you may need.

  • Drop-In Center

    FoundCare’s Drop-In Center offers social and emotional support for those who are infected and affected by HIV. It is currently offered at our Belle Glade and Boynton Beach offices on specific days (see information below). Drop-in days include Lunch and Learn Sessions or Free Days. Lunch and Learn Sessions include free lunch, health education, and information on medication adherence and treatment options. Free Days vary but may include meet-and-greet sessions or game days.

    PrEP, or pre-exposure prophylaxis, is a pill to help keep you HIV-negative. When taken as prescribed, PrEP is highly effective. PrEP is safe and generally well tolerated. PrEP works for women, men, people of transgendered experience, people of sexual orientations and gender identities, youth, and people who inject drugs. PrEP may be an option for you if:

    • You wonder how HIV impacts your life
    • Condoms are not used with partners of unknown HIV status
    • You or your partner(s) recently had gonorrhea or syphilis
    • You want to have sex without condoms with a partner who has HIV
    • You want to have a baby with a man living with HIV
    • You have sex for money, food, housing, and/or drugs
    • You inject drugs and share needles

    PrEP is highly effective in preventing HIV. PrEP reduces the risk of getting HIV from sex by 99% when taken as prescribed.

    How can I start PrEP?

    Talk to your healthcare provider if you think PrEP may be right for you. Before beginning PrEP,  a visit with a primary care provider will be required to assess your health (a fee may apply). You will also be required to take an HIV test to make sure you don’t have HIV before beginning PrEP. 

    What if I test positive for HIV at the time of my visit?

    FoundCare has over 35 years of experience in infectious disease care. If you test positive for HIV, our care team will help you navigate through your treatment options, provide education, and refer you to other support services to help you along your health journey. HIV is no longer a death sentence. You can live a long and healthy life with the proper medication, precautions, and care. 

    How much does PrEP cost?

    Most insurance plans and state Medicaid programs cover PrEP. Under the Affordable Care Act, PrEP must be free under almost all health insurance plans. That means you can’t be charged for your PrEP medication or the clinic visits and lab tests you need to maintain your prescription.

    If you don’t have insurance or Medicaid coverage, other programs provide PrEP for free or at a reduced cost. Your care team will help you find a program that is right for you based on what you qualify for. 

    PEP (post-exposure prophylaxis) means taking medicine to prevent HIV after a possible exposure. PEP should be used only in emergencies and must be started within 72 hours after recent possible exposure to HIV. Talk right away (within 72 hours) to your healthcare provider about PEP if you think you’ve recently been exposed to HIV:

    • During sex (for example, if the condom broke)
    • Through sharing needles, syringes, or other equipment to inject drugs
    • If you’ve been sexually assaulted

    The sooner you start PEP, the better. Every hour counts.

  • Mammography Screening

    The Promise Fund Mammography Screening Center provides a one-stop-shop for women to access breast and cervical cancer screenings coupled with other healthcare services necessary to achieve and maintain good health. A state-of-the-art 3D mammography unit, generously donated by Hologic, Inc., will provide free and discounted screenings to uninsured women with limited financial resources, thanks to funding from the Promise Fund of Florida. The screenings will serve as an enhancement to FoundCare’s existing Women’s Health Services.

    American Cancer Society Breast Cancer Screening Guidelines for Women

    The latest guideline applies to women at average risk for breast cancer. Among other recommendations, the American Cancer Society states all women should begin having yearly mammograms by age 40 and can change to having mammograms every other year beginning at age 55. Women should have the choice to start screening with yearly mammograms as early as age 40 if they want to.

    Preparing for Your Screening Mammogram: A Step-by-Step Guide

    How to Prepare for a Mammogram

    • If you are visiting FoundCare for the first time, bring a list of the places and dates of mammograms, biopsies, or any other breast procedures you’ve had before.
    • If you’ve had mammograms at another health center, try to get those records to bring with you (or have them sent to FoundCare) so the old pictures can be compared to the new ones. 
    • Schedule your mammogram for when your breasts aren't likely to be tender or swollen, to help reduce discomfort and get good pictures. Try to avoid the week just before your period.
    • On the day of the exam, don’t apply deodorant, antiperspirant, powders, lotions, creams, or perfumes under your arms, or on or under your breasts. Some of these contain substances that can show up on the x-ray as white spots. If you’re not going home after your exam, you might want to take your deodorant or antiperspirant with you to put on after your exam.
    • Wear a skirt or pants so that you will only need to remove your top and bra for the mammogram. 
    • Discuss any recent changes or problems in your breasts with your healthcare provider before getting the mammogram. 
    • Make sure your provider is aware of any part of your medical history that could affect your breast cancer risk—such as surgery, hormone use, breast cancer in your family, or if you’ve had breast cancer before.

    Informing Your Technologist

    To help ensure you have a good quality mammogram, make sure your technologist knows:

    • About any breast changes or problems you’re having
    • If you have breast implants
    • If you have trouble standing and holding still alone (without the aid of a cane or walker)
    • If you’re breastfeeding or if you think you might be pregnant.

    Tell the technologist right away if you start feeling lightheaded or dizzy during the mammogram.

    What to Expect During a Mammogram

    • You’ll have to undress above the waist to get a mammogram. You will receive a wrap to wear.
    • You and the technologist will be the only ones in the room during the mammogram.
    • To get a high-quality picture, your breast must be flattened or compressed. You'll stand in front of the machine, and the technologist will place your breast on the machine. The plastic upper plate is then lowered to compress your breast for about 10 to 15 seconds while the technologist takes an x-ray. You will then need to change position so your breast is compressed from side to side before the next x-ray is taken.
    • If you're getting a 3D mammogram (also known as digital breast tomosynthesis, or DBT), the procedure is the same as above, but you'll notice that the machine will move in a small arc, either over the top of your breast or along the side of your breast, for each image. You might be asked to hold your breath each time it's being done.  
    • The whole procedure takes about 20 minutes. The actual breast compression only lasts about 10 to 15 seconds for each image. 
    • You might feel some discomfort when your breasts are compressed, and for some women, it can be painful. Tell the technologist if it hurts so they can try to adjust the compression to your comfort.
    • Two views of each breast are taken for a screening mammogram. But for some women, such as those with breast implants or larger breasts, more pictures may be needed.

    A diagnostic mammogram is often done if a woman has breast symptoms or if something unusual is seen on a screening mammogram. Depending on the findings, a breast ultrasound may also be done to look at the area of concern.

  • Navigator Program

    FoundCare has partnered with the Promise Fund of Florida to pilot an innovative program designed to help people with breast and cervical cancer access critical services. Through a generous grant provided by the Promise Fund, FoundCare was able to designate a team of navigators to guide and support patients through the process of cancer screenings.

    The Promise Fund Navigator Program covers the full spectrum of care including education about breast and cervical cancer and linking people at risk to mammogram screenings and PAP smears. For those diagnosed with cancer, FoundCare’s navigators will identify the best resources to facilitate their treatment and meet other daily needs.

    One of the largest challenges faced when getting a diagnosis of cancer is not knowing where to turn for assistance. The Promise Fund has recognized this need in our community, and FoundCare’s navigators are ready to help guide and support patients through the process of accessing care and other services.

    Those diagnosed will receive follow-up services, as well as resources for such assistance as transportation to medical treatment and access to financial assistance for rent, food and other needs that can be strained by a cancer diagnosis. As a pilot program, FoundCare will work closely with the Promise Fund to collect data to document the services provided and document the large need for services, especially for those who are uninsured.

    FoundCare is currently working with existing patients who receive their primary care through our Palm Springs and North Palm Beach health centers. Many women at risk for breast and cervical cancer often come to FoundCare without a history of prior routine screenings.

    Diagnostic tests are used to confirm or rule out conditions and diseases. Before creating a treatment plan, your doctor needs critical information for a correct diagnosis, which a diagnostic test can provide.

    Eligibility Assistance and Navigation

    We know how difficult it can be to navigate the application process for benefits. We help those seeking to apply for assistance benefits by providing them with the information they need to understand the requirements of the program, learn about the application process, and help get the most out of their benefits.

    Digital X-Ray

    Digital radiography (digital X-ray) is a form of X-ray imaging where digital X-ray sensors are used instead of traditional photographic film. X-ray scans can diagnose possibly life-threatening conditions such as blocked blood vessels, bone cancer, and infections.

    Digital X-rays expose patients and staff to up to 90% less radiation than traditional film X-rays.

  • Testing

    Rapid HIV and HCV Testing

    Rapid HIV and Hepatitis C testing are offered at three of our FoundCare locations. Tests are free, confidential, and quick—results are provided within 20 minutes. Appointments are not required. FoundCare is also able to provide rapid testing offsite at organizations that ensure privacy and confidentiality.
