FoundCare Earns Two 2026 American Heart Association Gold Recognitions for Cardiovascular Care
Gold+ and Gold achievements recognize FoundCare’s commitment to blood pressure control, cholesterol management and high-quality cardiovascular care
FoundCare has earned two 2026 gold recognitions from the American Heart Association, highlighting the health center’s continued commitment to evidence-based care and improving cardiovascular health outcomes for patients.
The 2026 recognitions include:
- Target: BP™ Gold+
- Check. Change. Control. Cholesterol™ Gold
The Target: BP™ Gold+ achievement recognizes health care practices that have demonstrated evidence-based approaches to blood pressure measurement and treatment while achieving blood pressure control rates of 70% or higher among adult patients with hypertension.
FoundCare also earned Check. Change. Control. Cholesterol™ Gold recognition for its commitment to helping patients better understand, monitor and manage high cholesterol, an important modifiable risk factor for cardiovascular disease.
