FoundCare Earns Six HRSA 2025 Quality Recognition Badges for Excellence in Care

FoundCare is proud to announce it has been awarded six 2025 Community Health Quality Recognition (CHQR) badges by the Health Resources and Services Administration (HRSA). These distinctions honor health centers nationwide that demonstrate excellence in clinical quality, patient outcomes, access to care, and innovation.

This year, FoundCare earned recognition in the following categories:

1. Silver Badge - Health Center Quality Leader (Awarded to the top 11–20% of health centers nationwide)

2. Heart Health - National Quality Leader

3. Diabetes - National Quality Leader

4. Improving Access to Care

5. Advancing Health Information Technology (HIT) for Quality

6. High-Value Care

“These awards are a direct reflection of the dedication, skill, and compassion of our staff,” said Christopher Irizarry, Chief Executive Officer of FoundCare. “They show we are not only meeting the highest national standards for quality care but also making a meaningful difference in the lives of the patients we serve. We are grateful to our community for trusting us with their health.”

CHQR badges are awarded annually to HRSA-funded health centers based on performance data from the Uniform Data System (UDS). The badges recognize achievements in key areas such as preventive health, chronic disease management, technology adoption, and cost-effective care. In 2025, HRSA introduced two new distinctions, Preventive Health and High-Value Care, further emphasizing the importance of proactive, patient-centered care.
