Primary Purpose:
The primary purpose is to enroll patients needing social services in the Palm Beach County Connects system. The program ensures that the medically related and social needs of FoundCare patients are met/maintained on an individual basis.
ESSENTIAL JOB FUNCTIONS:
- Coordinate, Plan, and enroll FoundCare patients in Palm Beach Connects system.
- Process referrals received from the system, community partners; and provide follow-up as necessary.
- Process referrals received from FoundCare providers for patients in need of social services.
- Assist with referrals to other community resources.
- Interview patients to obtain social history information.
- Perform administrative requirements, such as completing necessary forms, reports, etc.,
- Involve the patient in planning a social service program when possible.
- Assist in arranging transportation to and from FoundCare and other facilities when necessary.
- Refer patients/families to appropriate social service agencies when necessary.
- Provide information to patients/families re Medicare/Medicaid and other financial assistance programs available to the patients.
- Provide consultation to members of our staff, community agencies, etc., in an effort to develop social service programs that will solve the needs and problems of patients.
- Maintain a quality working relationship with medical professionals, community partners, and organizations.
- Coordinate social services activities with other departments as necessary.
- Evaluate social and family information and assist in determining plans for social service interventions.
- Interpret social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other care team members.
- Assist in obtaining resources from community social, health, and welfare agencies to meet the patient's needs.
MINIMUM QUALIFICATIONS:
- High School Diploma including two years of customer service.
- Preferably, a bachelor’s degree in social work or a human services field.
