Due to the high demand for our Back-to-School event, we, unfortunately, have to limit the number of children we can provide services to.
Our new registration process will help us speed up the flow of the event, eliminate the need for families to have to wait out in the hot sun for long periods of time, and allow our providers and team to focus on providing a quality experience. We hope you will notice a difference!
If you have multiple children you plan to bring to the event, each child MUST have their own registration form. One form for multiple children will NOT be accepted.
For those who cannot register online, our team is available to help. Please call 561-432-5849. We also have a team of outreach workers who will be out in the community registering families for the event.
If your children are current FoundCare patients, we ask that you please schedule an appointment for a different day so we may provide these services to those who need it most.
