All school-aged children will receive the following services free of charge: 

• Hearing and Vision Screenings
• School Physical
• Vaccines
• Free Backpack w/ School Supplies

FoundCare's Back-to-School Health Fair does not cost anything to attend, however, this year, children must be pre-registered. Our event was built to ensure school-aged children are fully prepared for the new academic year without the added stress and financial burden on their families.

Due to the high demand for our Back-to-School event, we, unfortunately, have to limit the number of children we can provide services to.  

Our new registration process will help us speed up the flow of the event, eliminate the need for families to have to wait out in the hot sun for long periods of time, and allow our providers and team to focus on providing a quality experience. We hope you will notice a difference!

If you have multiple children you plan to bring to the event, each child MUST have their own registration form. One form for multiple children will NOT be accepted. 

For those who cannot register online, our team is available to help. Please call 561-432-5849. We also have a team of outreach workers who will be out in the community registering families for the event. 

If your children are current FoundCare patients, we ask that you please schedule an appointment for a different day so we may provide these services to those who need it most. 

We cannot guarantee service to those who do not register and/or missed our registration period.

We will have very limited walk-ins available per hour. Should you like to try to attend as a walk-in, we cannot guarantee service will be available and may have to turn you away.

Backpacks are reserved for registered participants only. There are no exceptions to this. Anyone who arrives at the event requesting backpacks will unfortunately have to be turned away. 

We are asking parents/guardians to upload their children's shot records (immunization forms) ahead of time so that we may translate, and verify the information ahead of time. Should you choose not to do so, we will provide a copy to you on-site. Please note, this may require you to wait longer for services. 

Parking will be available for free throughout Congress Park. FoundCare's main lot will be closed.
