FoundCare's mission is to fulfill unmet healthcare and social service needs of individuals and families in Palm Beach County. In order to accomplish this mission, FoundCare operates a Federally Qualified Health Center that offers pediatric and adult primary care, chronic disease management, infusions, behavioral health counseling, laboratory work, X-rays and dentistry, as well as an on-site pharmacy.

Position Summary

We are seeking an enthusiastic, detail-oriented, self-motivated professional to fulfill key functions as our Peer Advocate Liaison. Implement behavioral health outreach activities for HIV/Hepatitis patients and/or patients at risk of HIV/Hepatitis, mostly in minority and underserved communities. This will include outreach to the community to increase mental health, substance use awareness, condom distribution, and rapid testing clients for HIV and Hepatitis, as well as referring clients at risk to FoundCare clinic. Providing navigational and support services to HIV positive and high risk negative individuals. This includes information on risk-reduction methods, vaccination about Hepatitis A & B, medication adherence, linkage, referrals and the importance of mental health.

Essential Job Functions  

  • Reach out to minority communities (LGBTQ, African-Americans, Hispanics, Haitians, Caribbeans) to provide information about HIV and Hepatitis, as well as information about mental health hygiene.
  • Provide HIV/Hepatitis Counseling Testing and Referral program activities, including assurance of compliance with state and agency standards and maintenance of quality assurance protocols.
  • Gather demographic data about clients reached, including the outcome of educational sessions with clients.
  • Provide navigational and support services to individuals positive for HIV and/or Hepatitis and maintain the participant’s files and records.
  • Present at the monthly Drop-in centers. Provide educational and support services to clients in the Drop-in center.
  • Identify/conduct community street outreach for targeted Palm Beach County minority communities and establish partnerships between FoundCare and community partners. This includes having at least 50 face to face contacts and conducting at least 10 rapid HIV tests each month.
  • Distribute prevention materials and condoms to clients during events.
  • Provide required information for completion of monthly, quarterly, and annual reports as well as preparation for contract monitoring site visits.
  • Maintain current knowledge of HIV/STD risk reduction strategies.
  • Notify the Director of Behavioral Health of factors that inhibit successful completion of program deliverables and make recommendations for implementing measures to reduce/eliminate those factors.
  • Responsible for participating in HIV/AIDS Awareness activities including but not limited to; National HIV Testing Day, World AIDS Day, and Black HIV/AIDS Awareness Day.
  • Having the flexibility to work weekend and evenings hours as required.
  • Responsible to contributing to ensure program monthly and annual goals are met.
  • Assist in the program development and implementation plus work in conjunction with the HIV Prevention Supervisor to implement the program policies and procedures.
  • Facilitate group educational sessions, as needed.
  • Attend Department and program trainings and meetings as assigned.
  • Maintain client and family confidentiality.
  • Provide culturally sensitive information to clients, while respecting their cultural perspectives.

Required Knowledge, Skills and Abilities

  • High School Diploma/GED
  • Minimum 1-2 years’ experience in a customer / direct public service setting OR Associate degree in Social Services/Education/Health Care related field.
  • Experience providing HIV prevention services preferred. Knowledge of HIV care continuum and services, as well as some knowledge of mental health symptoms.
  • Current knowledge of HIV/AIDS, Hepatitis and other STDs, disease transmission and HIV/AIDS and prevention strategies.
  • General knowledge about mental health.
  • Ability to verbally communicate effectively with others, with or without the use of an interpreter.
  • Ability to communicate effectively in writing using the English language, with or without the use of auxiliary aids or services.
  • Bi or Tri-Lingual a plus, (Creole French & Spanish)
  • Open minded and receptive to the LGBTQ community.
  • Ability to review, understand, and apply concepts presented in training programs, conferences, and professional literature.
  • Ability to educate individuals on safe sex practices which includes but not limited to, exposure to explicit language, explicit printed material, and descriptions of explicit sexual activities as part of the agency’s mission in the prevention and transmission of HIV and hepatitis.
  • Knowledge on the dynamics of a diverse population, including verbal and written skills adaptable to diverse audiences.
  • PC Proficient (Word, Excel, Outlook, PowerPoint).
  • Willingness and ability to work flexible schedule, including evenings or weekends.
  • Valid Florida driver's license, automobile insurance and reliable automobile.

 Physical Requirements

  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Accomplish job duties using various types of equipment/supplies, e.g. pens, pencils, calculators, computer keyboard, telephone, etc.
  • Ability to lift and carry objects weighing 25 pounds or less over distances of 100 yards.
  • Ability to move, stack, and store department materials and supplies in storage area, including lifting materials and placing on shelves at levels 5’ 6” and lower.
  • Transport him/herself to other departments, conference rooms, and on occasion to off-site locations to accomplish job responsibilities, attend meetings, workshops, seminars, etc.
