FoundCare's mission is to fulfill unmet healthcare and social service needs of individuals and families in Palm Beach County. In order to accomplish this mission, FoundCare operates a Federally Qualified Health Center that offers pediatric and adult primary care, chronic disease management, infusions, behavioral health counseling, laboratory work, X-rays and dentistry, as well as an on-site pharmacy.

Position Summary

Implement Peer Navigation services within Palm Beach County community. This will include coordinating the Drop-In Center, providing navigation and support services to people living with HIV, and providing information on risk-reduction methods, medication adherence, linkage, and referrals.

Essential Job Functions

  • Provide education, navigation and support services to individuals living with HIV.
  • Coordinate Drop-in Center activities and maintain participants’ files and records.
  • Actively participate in Drop-In Center recruitment activities.
  • Provide required information for completion of monthly, quarterly, and annual reports as well as preparation for contract monitoring site visits.
  • Maintain current knowledge of HIV/STD risk reduction strategies.
  • Notify the Health Education Manager of factors that inhibit successful completion of program deliverables and make recommendations for implementing measures to reduce/eliminate those factors.
  • Participate in HIV/AIDS Awareness activities including but not limited to; National HIV Testing Day, World AIDS Day, and Black HIV/AIDS Awareness Day.
  • Flexibility to work weekend and evenings hours.
  • Responsible to contributing to ensure program monthly and annual goals are met.
  • Attend Department and program trainings and meetings as assigned.
  • Maintain client and family confidentiality.
  • Assist with agency fundraising activities as requested, including promoting events, selling tickets, soliciting donations and other tasks to support events.
  • Conduct rapid testing, counseling, and linkages (CTL) according to Department of Health (DOH) guidelines.
  • Other duties as assigned.

Required Knowledge, Skills and Abilities

  • High School Diploma/GED and 1 year experience in a customer / direct public service setting OR Associate degree in Social Services/Education/Health Care related field.
  • Valid Florida driver's license, automobile insurance and reliable automobile.
  • Ability to communicate with clients, families and staff from community agencies and engender cooperation.
  • Current knowledge of HIV/AIDS and the ability to communicate same to clients, significant others, subordinates, and persons in the community.
  • Current knowledge of HIV/AIDS and other STDs, disease transmission and HIV/AIDS and prevention strategies.
  • Knowledge of the dynamics of a diverse population.
  • PC Proficient (Word, Excel, Outlook, PowerPoint).
  • Must be a person living with HIV whose open with their status and their life experience.
  • Knowledge of HIV care continuum and services.
  • Ability to verbally communicate effectively with others.
  • Ability to communicate effectively in writing using the English language.
  • Ability to educate individuals on safe sex practices which includes but not limited to, exposure to explicit language, explicit printed material, and descriptions of explicit sexual activities as part of the agency’s mission in the prevention and transmission of the HIV.

 Physical Requirements

  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Accomplish job duties using various types of equipment/supplies e.g. pens, pencils, calculators, computer, telephone, etc.
  • Ability to lift and carry objects weighing 25 pounds or less over periods of 100 yards.
  • Ability to move, stack, and Prevention Department materials and supplies in storage area, including lifting materials and placing on shelves at levels 5’ 6” and lower.
  • Ability to transport him/herself to meetings, trainings, and other off-site locations needed to accomplish job responsibilities.
